Pioneer Medical Center is a 25-bed critical access hospital, essentially the county hospital for Sweet Grass County, Montana: Population 3700. Pioneer Medical Center has an immediate opening for a full-time Outpatient/Clinic/Cardiac Rehab Nurse to join our hospital, emergency & clinics' patient care team.
PURPOSE/DISTINGUISHING CHARACTERISTICS:
Under general supervision, utilizes the nursing process of assessment, planning, implementation and evaluation, and theoretical/clinical skills in the promotion/maintenance of health. Incorporates evidence-based nursing practices consistent with national standards and which support quality outcomes. Assumes responsibility and accountability for the quality of nursing care rendered.

Overview:
We are seeking a highly skilled and compassionate Nurse (RN/LPN) to join our team. This multifaceted role involves providing care in the outpatient clinic, leading the outpatient procedure program, coordinating our cardiac rehabilitation program, and serving as a liaison for Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) programs. The ideal candidate will have excellent clinical and organizational skills, a passion for patient care, and a commitment to improving health outcomes in our community.
Key Responsibilities:
Supports and models behaviors consistent with Pioneer Medical Center’s mission, vision, values, code of business conduct and service expectations. Meets all mandatory organizational and departmental requirements. Maintains competency in all organizational, departmental, and outside agency standards as it relates to the environment, employee, patient safety or job performance.
Outpatient Clinic:
• Provide direct patient care, including assessments, treatment, and follow-up for a variety of outpatient conditions.
• Assist providers with clinical procedures, exams, and treatments.
• Educate patients and families on health conditions, treatment plans, and preventive care.
• Ensure proper documentation of patient encounters in the electronic medical record (EMR) system.
Outpatient Procedure Program:
• Coordinate and oversee the hospital’s outpatient procedure program, including scheduling, pre-procedure preparation, and post-procedure follow-up.
• Collaborate with providers and multidisciplinary teams to ensure procedural protocols are followed.
• Monitor patient outcomes and implement improvements to enhance program quality and efficiency.
• Train and mentor staff involved in the outpatient procedure program.
• Develop, review and implement policy and procedures for outpatient program.
Cardiac Rehabilitation Program:
• Lead and manage the cardiac rehabilitation program, providing individualized care plans for patients recovering from cardiac events.
• Conduct patient assessments to determine readiness for rehab and tailor exercise and education plans accordingly.
• Monitor patients during rehab sessions, ensuring safety and adherence to protocols.
• Educate patients and families about lifestyle changes to improve cardiac health.
• Collaborate with cardiologists and other healthcare professionals to optimize patient outcomes.
• Drive data driven outcomes to elevate patient care.
Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM):
• Act as the primary liaison for CCM, TCM, and RPM patients and programs.
Performs other duties as assigned or needed to meet the needs of the department/organization.
Minimum Qualifications:
• Active RN/LPN license in MT.
• Bachelor of Science in Nursing (BSN) preferred.
• At least 3 years of nursing experience, with experience in outpatient care, cardiac rehab, or program coordination strongly preferred.
• Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certifications required.
• Experience with Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) programs is highly desirable.
• Strong leadership, organizational, and communication skills.
• Proficiency with electronic medical record systems and remote monitoring platforms.
Working Conditions:
• Normal patient care environment in a physician clinic/outpatient setting
• Utilization of personal computers for financial and clinical database systems
