What You'll Do:
Be a Guiding Light: Provide personalized medical case management to injured workers through in-person and telephonic communication.
Collaborate and Advocate: Work closely with patients, their families, medical providers, and our team to facilitate optimal recovery.
Optimize Treatment Plans: Use your medical knowledge to evaluate treatment plans, discuss alternatives with physicians, and ensure appropriateness, necessity, and cost-effectiveness.
Coordinate Care: Plan, implement, and evaluate patient progress, ensuring seamless transitions and comprehensive care.
Connect In-Person: Attend doctor's appointments, home visits, and hospital discharge planning conferences to advocate for your patients' needs.
Facilitate Resources: Negotiate the delivery of durable medical equipment and nursing services to support patient recovery.
Enjoy Flexibility: Work from the comfort of your home and travel locally to meet with patients and providers (approximately 60% travel).
Occasional overnight travel may be required.
What You'll Bring:
Effective communication and multitasking skills in a dynamic, team-oriented environment.
Experience as an RN Medical Case Manager, or a clinical background in orthopedics, neurology, or rehabilitation.
Ability to build rapport with patients, physicians, and other healthcare professionals.
Knowledge of cost containment strategies, such as utilization review or managed care (helpful).
Strong interpersonal, time management, and organizational skills.
Proficiency in MS Office, including Excel.
Ability to work independently and as part of a team.
Graduate of an accredited school of nursing.
Current RN licensure in the state of operation.
Certification as a CCM, CIRS, or other case management certifications (preferred).
Valid driver's license, reliable transportation, and ability to travel.
